Controller Controls

I somehow reset my controls for my controller, and they were all messed up. SO I tried to set it up again, but nothing was working how I wanted them to. I've spent an hour on this, and it's just not working. It's assigning random controls on my controller, and nothing is saving or working right. What could I possibly be doing wrong?

 

Could it be you are forgetting to click on 'apply' after assigning the control ? I believe some need to be applied
and others not. Not sure but that's first thing coming to mind

I think it is ZD-V+. I didn't have trouble setting it up the first time, but now it isn't working right.
