3090 Ti getting sub 20 fps

I am playing the game maxed out at 4k, but the gpu is only at ~40% use and I average about 18fps, is that just the way it works or is this a client sided issue?

What does the CTRL SHIFT F debug menu show? Also does it run better in Vulkan?

If the gpu is 40%, is the cpu around 90 or 100%?
