    I'm having issues with using a controller with a headset. if the controller shuts off (my cord is junk) then i turn it back on. The game hangs then crashes i believe it has something to do with the audio stream switching back and forth from, headset to speaker mode.

    If the controller is shutting off, the audio device is also switching to something else because the headset won't be available anymore.
    Switching audio device while the game is running is not recommended/stable as of now.

    Strangely enough switching my audio source before the second hotfix wouldn't work for me, but ever since then I have had no issues... Have fixes been made or is this coincidental?
