Alpha Lock-In Dash 1.4

• Checked against the v0.40 release notes. No breaking changes affect the app, so it should run as before.
• The HUD now resets when you leave the vehicle or have no vehicle (for example in walking mode). It shows 0 speed, 0 RPM, an empty RPM bar and - for gear, instead of freezing on the last values.
• Added a console warning if the game’s stream system hasn’t loaded after about 10 seconds, to make startup problems easier to diagnose.
• Both background timers are now cancelled...

• Gear display now works on automatic transmissions. It shows P, R, N, D, S and M-modes instead of being stuck on "N".
• The RPM bar now scales to each engine's actual max RPM instead of a fixed 7000, so diesels and low-revving engines fill the bar correctly.
• Removed the duplicate stylesheet, so there is now one source for the app's styling.
• Version number is consistent between the app and the mod listing.

Fixed: App would fail to load at boot with Unknown provider: StreamsManagerProvider <- StreamsManager <- lockInDashDirective, permanently killing the directive for the session. StreamsManager was being injected as a hard dependency in the directive's DI array, which Angular tried to resolve before the streams module had registered it. Replaced with a lazy $injector.has('StreamsManager') poll (retrying every 200ms) so the directive registers successfully regardless of boot order.
