The "10-Speed Eaton Fuller Automatic Transmission" used Allison TC10 gear ratio and the Eaton "automated manual" cannot start at 2nd gear or above. I can lend a hand to fix that, using my(currently WIP) automated manual transmission logic mod.

Pretty sure neither trucks or cars can starts at fifth gear. (Idk if it's different with heavy duty semi trucks, I don't know anything about gearboxes work)

You can pull away in 2nd or above. Sometimes it's useful on slippery surface(lower torque) or very light loads(skip gear)

 

Did this B-series ever had a cargo box? I saw some guy on reddit that posted a picture of the B-series with a white cargo box.

 

No. Probably the older version of the mod, it used to be a front clip and bumper for the T series.

I do it on ETS2, but I wasn't sure if it wasn't just on of the very few arcade unrealistic aspects of the game

You are probably using ATO2612F ASO-ULC. The C1 and C2 are Crawler gears, useful in very slow movement or hauling very heavy loads. More iniformation
Also if you switch to arcade mode, the computer assists may shift to 4th gear at start when unloaded.
