safety valves in essense are a flap that is spring loaded shut. Steam pressure wants to force the flap open, the spring holds it closed and keeps the pressure. The only failure state for such a valve is for the spring to fail and the flap to be able to move freely, venting all pressure. Boilers without safety valves explode yes, boilers with, dont. The failure state for any safety device should be the safe state. Its why big rig brakes actually require pneumatic pressure to release the parking brake not engage it, airlines rupture etc you have brake.
