My parents were a bit strict and wouldn't let me run against the cops for a good while, so I was older the first time I saw that. Also, it's quite likely that I instinctively knew the helicopter stuff was something to avoid, so it was probably less of a shock the first time one of the barrels blew up in my face.
Back when i played world of tanks i had something go wrong, and i submitted a ticket and the company had to log onto my account to investigate.

Minecraft. I was scared stiff by an enderman, and i was so traumatized that I deleted the server. Eventually i mustered my courage and came back, but still... --- Post updated --- And also, Roblox when I got a warning. I said ISIS SUCKS but Roblox said that I said ISLAM SUCKS which is false.
The hospital in Walking Dead Survival Instinct. I know the game sucks ass etc etc, but the first time I played this it was pretty scary.
Once I nearly broke my Xbox 360... I got NFS rivals, upgraded roadblocks and helicopters to level 4, started a hot pursuit, and spammed the tech. Let's just say that the Xbox is not built for 60+ cars on screen at once.

Another thing that was really scary when i played modded minecraft with some friends and the alarm from our reactor went off and all cables blew up because of that. (Was a giant explosion with nuklear meltdown sadly the whole map was broken then

I was in a 5 star chase on the crew in a slow car off road and almost hit a train, but the cops got stuck at the other side of the train as it passed i missed it by less than an inch XD I escaped the helicopter too once i got on road
My scariest moment in a video game was when I got the 5th notebook in Baldi's Basics. After I answered the 3 questions from getting the notebook, Baldi was right behind me.
The most scary moment in a video game is when i played too much fnaf I got so scared I legit thought Foxy was in my house
Being serious, i almost become sometime a f*cking RAMPLOW when i was a bit dizzy(not really drank something, was tired as the utter hell), i was at a highway, i didn't see a car and rammed it, not really one, almost RAMPLOWED all the lane(i play with car limits, yeah XD)
