1955 Dune Island Air disasterOn the 27th of October / 1955, 2 planes disappeared while flying above Dune Island. Some small wreckage fell to the the island's streets killing 7 people and injuring 14 people badly out of 24. Flight a Crash : 'it was a routine daily flight above Dune Island' a Pilot of a B-25 said. On the 27th of October / 1955, 2 USAF planes exploded above Dune Island at around 5 AM. "The Sun was rising. Suddenly, we saw some debris falling from the sky" a woman said. Some parts of both planes fell into Dune Island streets killing 7 people and injuring 24 others. Emergency services arrived seconds after reports of a plane crash. At around 6 AM emergency services managed to remove all wreckage from the streets of the Island and moved them to a hangar where they were investigated. At around 6:43 AM investigators said that the recovered wreckage was only 10% of both planes. Boats were sent to the sea to see if there is any wreckage but the only thing they found were some glass debris and the elevator of the Cessna 305 prototype. - Accident Summary - Site : Above Dune Island / USA Date : 27th Of October / 1955 Summary : Possible Explosion Decompression and lightning strike Total Fatalities : 11 Total Survivors : 14 Total injures : 24 (10 minor/ 14 Serious) First Plane : (A Cessna modal 305 prototype similar to the one involved) Aircraft type : Cessna L19 Modal 305 prototype Crew : 2 Fatalities : 2 Survivors : 0 Operator : USAF Route : Nevada Military Airfield To Dune Island Airfield Registration : 35HII 2nd plane : (The L19 involved seen at Ghost's desert on a night flight) Aircraft type : Cessna L19 Crew : 2 Fatalities : 2 Survivors : 0 Route : Unknown To Dune Island Airfield Registration : 36HII - - After 16 days of searching the search was cancelled. The investigators had to find what caused both planes to explode with only 15% Of the wreckage. Investigation : The investigators were on a hurry because they were given 60 days to solve the mystery. One investigator pointed weather could have brought down the plane or some lightning strike. The team got interested on the weather theory so they visited the Island to ask about the weather at the time of the crash. Further more, they got the report of around 50 eyewitnesses. After hours of working they discovered that both planes entered Dune Island on an altitude of 5280 (1 mile exactly) not knowing there was fog and a nearby storm. The planes were not seen when they entered the big cloud of fog. Now what happened on the cloud of fog is unknown, the planes might hit each other or lightning strike brought down both planes but there is one missing evidence. The wreckage. Both planes wreckage were on an area of approximately 254 yards (232 meters) which is very large for a wreckage of a falling plane. The investigation team later said both planes exploded because of decompression. The reason for the explosion is unknown so this disaster is a Cold Case. In memory of all people who lost their lives on this mad-made written story. And in memory of Narwhal. RIP--- Post updated --- 1992 Jungle Rock Island Mid air collision On the 3rd of May / 1997, a restored Cessna L19 Collided with a Strato HMX 920. The strato was destroyed on impact but the L19 managed to crash land at Jungle Rock Island Airfield. __________________________________________________________________________ Flights / Collision : -Accident Report- Site : Near the lighthouse / Jungle Rock Island Date : 3rd Of May / 1997 Summary : Pilot error led to a Mid Air Collision Fatalities : 1 Survivors : 2 First Aircraft : (The plane involved seen back on 1959 on the same airfield) Aircraft type : Cessna L19 Bird Dog Crew : 2 Survivors : 2 injures (non-fatal) : 2 Operator : Historical Flying Museum Registration : 36 ☆ MAR _______________________________________________________________________ Second Aircraft : Aircraft type : Strato HMX 920 Crew : 1 Survivors : 0 Fatalities : 1 Owner : Privately Owned by the Captain Registration : HH185 _______________________________________________________________________ The first aircraft, a very old Cessna L19 that was made back in 1953 was on a flight to take it to the Museum of Air located on Utah. The plane had 2 experienced pilots with over 20.000 flying hours. The 2nd aircraft cas a newly made Strato HMX 920. The plane was only 2 years old (made in 1995). The plane was bought by a training pilot who aged 54 on the time of the crash. He died from the impact. On the 3rd of may / 1997 these 2 planes collided over the sea of Jungle Rock island causing the Strato to get wrecked completely. On the other hand, the Cessna manged to crash land on the nearest Airfield. The reason why both planes came in one point was unknown. The Jungle Air investigators said that due to the night and clouds both planes couldn't see each other which caused the collision. But the a team of Strato investigators went to see what happened and said that the Strato had a CVR and a TCAS on the time of the crash. So all investigators wanted to get both the TCAS instrument and the CVR. After 2 days of searching on the deep floor of the jungle's water, Both the CVR and the TCAS instrument were found. The CVR was realized to the public after some days of the recovery. Here are the last words by the pilot : CA (Captain) : The weather is nice isn't it, haha ! *Captain calls the ATC to see if there is any traffic* Ca : HH185 asking if there is any traffic near the Lighthouse. ATC : HH185 Traffic is cleared but there is a plane going fast right next to the light house. Ca : Which side ? ATC : Its Coming straight at you but on an altitude of 3000. Ca : Roger that. Asking to maintain level 2000 ATC : HH185 You are cleared to do that, what altitude are you in right now ? Ca : 2800 Sir. ATC : Roger that HH185, Good night ! Ca : Good night ! *TCAS warning starts* TCAS : Traffic Traffic Ca : What the hell ? TCAS : Climb Climb Climb Ca : OH SH*T *Sounds of impact* -Sounds of debris falling from the sky- The investigators could hear the L19 engine on the last 10 seconds of the recording but for some reason the Strato Pilot couldn't. Investigators found that the TCAS was telling the pilot to climb but the pilot descended insisted. The impact was so deadly that the Captain of the Strato was killed instantly, his recovered body was cut completely. The Crash cause was later determined to be Pilot error on the Strato. ________________________________________ Photo Gallery : This photo was realized on 2000. USAF Flight Number 199USAF flight Number 199 was the designation of 10 Cessna O-1 Bird dogs that were flying above Green Valley on the 2nd of June / 1951. Suddenly one of the Bird dogs lost control and went on a steep dive to the ground. After 15m of struggling the pilot of the Cessna manged to ditch on the Green Valley lake. He and his Co-pilot survived but the plane was written off. ____________________________________________________________ Facts / Events : (The L19 after the perfect ditching / 1951 ) Date : 2nd of June / 1951 Site : Green Valley Summary : Jackscrew failure due to unknown reasons, possible metal rust Crew : 2 Survivors : 2 Injures (Non-fatal) : 2 Plane type : Cessna L19 Bird Dog Registration : USAF Flight 199 Operator : USAF Route : Unknown To Unknown The plane was a 1 year old brand new Cessna O-1 Bird Dog. The plane had 500 flying hours with no problems reported. The crew were 2 USAF experienced pilots. The Captain, George Sherlock had over 9000 flying hours while the Co-pilot, first officer Mike had 8500 flying hours. On the 2nd of June / 1951 the plane was a part of USAF Flight 199, a flight number given for 10 Bird dogs that were flying above Green Valley. At 7:12 AM the plane went out of the flight plan, the other bird dogs thought that Captain George only moved the the lever to the right by mistake but it was much much worse than that. Captain George called one of the planes using his radio telling him that he lost control and the plane is not responding at all. That was the last massage received before the plane dived to the ground dangerously. The other planes crew thought that this is the end for Captain George and his Co-Pilot. But, that wasn't the end at all. Both crew manged to control the plane at the last few seconds. They were struggling for control as much as possible after they came out of the dive. Fortunately, they got control back ,but that wasn't the end. The plane went into a 2nd steep dive. This time the Captain decided to ditch at The Green Valley lake. Thankfully he made a successful ditching, both crew members were rescued by near fishing boats. Unfortunately, the L19 was pretty badly damaged because of the ditching, but it was recovered than it was sent into a place where investigators examined the wreckage. _______________________________________________________________ Investigation : The US military gathered a team of investigators to see what caused the crew to loss control. It was found that the Jackscrew of the tail failed due to unknown reasons, investigators said : "Its possible that the failure was caused by metal rust". _______________________________________________________________ Photo Gallery : The wreckage of the L19 being transported. 4th of June / 1951.

Combined with dark and foggy night the crash was unavoidable. __________________________________________ Photo gallery : The accident.
N60012 Crash / 1972 Nevada Private Plane CrashN60012 was the registration for a private Socata ST-10 that was flying above the Nevada Interstate. On the 24th of August / 1972 the plane broke up in mid-air while flying to the airfield near the wind farm. All people on bored perished. It is the worst accident involving a Socata ST 10 In Nevada. ____________________________________________________________________ Plane and explosion : The plane was a 9 years old Socata ST-10 With around 19.564 flying hours. The Socata was used as a private plane 2 years before the crash. The pilot was an normal pilot with 4000 flying hours. 2 passengers with 2 children where on the back seat while there was another passenger next to Captain seat. The Socata was registered as N60012. Events : At 6 AM after sunrise, the plane asked the ATC of the big military airfield located north of the small airfield (that the plane flying to) if they can fly on 2000 feet. The ATC approved their request but he told them that there is some clouds and wind shear. 5 minutes later, a police officer reported seeing a wing falling from the sky. He said the plane was for sure for a light aircraft. 2 minutes after the officer report, 9 people driving on the Nevada interstate reported seeing wreckage falling from the sky towards the highway. The Nevada police knew something bad happened so they sent emergency services to the reports site. They sent a team of investigators to exam the reported wreckage. Finally at 6:21 AM the emergency services managed to arrive. They discovered the wing of a plane upside a car with 4 people inside it, everyone on that car were killed. At 6:28 the ATC tower on the Big military airfield was informed about the plane crash. Air traffic controllers started to check for a missing aircraft, they couldn't find anything until 6:35 AM. they knew the missing aircraft. It was registered as N60012. Investigators said the plane was a Socata ST-10. Investigation : (The plane's wreckage dropping from the sky) Date : 24th of August / 1972 Site : Nevada Interstate Summary : Mid-air break up caused by Pilot error. Details of the Summary : Failure in communications let the plane to enter a cloudy area with wake turbulence. Combined with bad maintenance, the plane broke up after a sudden steep drop by the pilot. Crew : 1 Passengers : 5 (2 children) Fatalities : 10 (All on bored including 4 on the ground) Survivors : 0 Injures (non-fatal) : 4 on the ground (Due to shock) Registration : N60012 Operator : Private plane. The team of investigators were puzzled how can a light aircraft break up in mid-air in such a normal day. They investigators asked for all ATC calls as the plane's CVR was so badly damaged. The next morning, all ATC calls were collected. After examining the calls for 1 week the investigators found a shocking discovery. The Pilot of the Socata didn't say at what altitude he was flying at. They even discovered the ATC had an old radar that can't track planes altitudes. They realized the calls 2 weeks after examining them. Here are the most important parts : CA (Captain) : Nevada Military airbase, here is N60012 Good morning sir. ATC : N60012 Welcome sir how I can help you ? CA : We are asking to maintain level 2000. ATC : N60012 You are cleared to do that. Anything else ? CA : No thank you. N60012 maintaining level 2000. Have a nice day. ATC : You 2 sir. CA : Damn, 2 many clouds today. Passenger : Yep. *Sounds of cracks* CA : The hell was that ? Passenger : I Don't know. CA : I want to land as fast as possible. Lets drop even more. *Unknown because the CVR started to jam due to the bad drop* CA : MAYDAY MAYDAY ! ATC : Did you hear that ? Someone said mayday. "Possible" CA : What . . . Is this "Possible" CA : That's it people, we are going to die. *Sounds of the plane hitting the ground* Discoveries : The investigators determined the cause of the Mid-Air break up as a break up caused by turbulence and the sudden deep drop by the pilot. The team also found that the last maintenance done to the plane was 3 years ago. That caused the plane's fuselage to craze day by day. The pilot was blamed for the crash. The Team made this report about his actions : Failure in communications Dropped the plane's nose to steep which caused the plane to break up. ___________________________________________________________________________________
6th of October / 1956 Utah Air Crashes (Also known as the "Black Day of Utah")The 6th of October / 1956 was a normal day just like any day on Utah. Suddenly, the people of Utah were surprised by an uncommon visitor. The visitor was a bad thunderstorm that changed the day completely. "Heavy rain, dark clouds even lightning" One of the police officers said. At 6:32 O'clock, 2 planes operated by the USAF were flying above Utah when they suddenly entered the storm. The first plane crashed but the 2nd one manged to crash land. After 3h, a Cessna 172 made a crash landing on the West Highway. Both occupants survived. _____________________________________________________________ Details : First Plane : A C-Titan (Upgraded C-130) Operated by the USAF with 5 crew members and 36 Military troops was flying from Colorado to Nevada. The plane was a new Titan with at least 2000 flying hours. The crew were experienced. Everyone had over 6000 flying hours on different Military cargo planes. Second Plane : The 2nd plane was an old B-25 Operated by the USAF. With over 20000 flying hours and serviced on the WW2, the plane was every pilot's dream. A dream came true for 3 old crew members. They all serviced on WW2 and were very experienced. The pilot, Captain James had over 17100 flying hours. First officer Hanson B. Marks had over 15600 flying hours. The 3rd crew member was Flight engineer Jack Pete. He had over 16000 flying hours. Third Plane : The 3rd plane was a brand new Cessna 172 with only 20 flying hours. 1950 B-25 Nevada CrashOn the 6th of October / 1950, a B-25 operated by the USAF Crashed 9 minutes after take off due to in flight fire. The fire source was never determined. All crew on board survived but with critical injures. _______________________________________ Background : A B-25 Mitchell bomber owned by the USAF wanted to make a bomb test above the Nevada desert. The B-25 Was 4 years old with 3000 flying hours. The test flight was piloted by Captain Josh Rick, he had over 8500 flying hours. On the other hand, the 2nd pilot, first officer Captain Fredrick T John had 5000 flying hours. The other 3 crew were some North American Aviation flight engineers. The plane toke off at around 5:22 AM. Everything seemed normal until one of the flight engineers reported a fire in the bomb cargo hold. Captain Josh wanted to return to the airport but his controls were badly damaged by the fire, he decided to crash land anywhere near. These are the last words from the cockpit depending on the ATC calls with the plane before the it crashed : - Capt Josh : OK OK. I have it, lets crash land over there. - FO (first officer) Fredrick : I will call the tower, - Capt : Roger. - FO : "Tower, here is 973. We are having a fire and we are going to crash land" - ATC : "973, roger. Can you tell me where you are going to crash land" - Unknown (Possibly the 3rd flight engineer) : Josh, we lost a gear - FO : God damn. - 2nd flight engineer : Pull it up ! - Capt : I'm trying ! - FO : Brace for impact guys. - "Screams" *Sound of the first explosion* - 1st flight engineer : Sh*t guys, I think we are done *2nd explosion that destroyed the plane* The plane crashed into a mountain 9 minutes after take off. Emergency workers arrived half a hour later. From the crash site, they thought there were no survivors , but they found out that all the crew on board were on the cockpit when the plane crashed. The cockpit wasn't damaged a lot from the crash which made the survival rate on the front part of the plane better. All the 5 crew were rescued. They all had critical injures but no one died. ___________________________________________________________________________ Cause : (a B-25 Similar to the one involved in the accident) Date : 5th of October / 1950 Site : Nevada Summary : In-flight fire. Cause never determined Aircraft Type : North American B-25 Mitchell Bomber Operator : USAF Crew : 5 Survivors : 5 (all) Injures (non-fatal) : 5 Fatalities : 0 Phase : Bomb test flight Airplane fate : Written off___________ A team of experienced investigators were gathered on the crash site. They discovered that the plane's right landing gear was destroyed when it touched down. That caused the plane to lose balance and caused the bombs on board to get badly damaged. The captain tried to use the Rudder to help him stopping , but the bomb cargo exploded causing the plane to chatter into pieces. The 2nd explosion happened when the Gases out of the bomb cargo mixed with the fire causing another deadly explosion. The investigators never determined the cause of the fire but they have 2 possible causes : Possible wire system failure and electric fire which caused the cargo tank to ignite. Possible Bomb-failure. The investigators think that one of the bombs on board the plane wasn't made very well. They think that the engineers who made the bomb forget to cover the whole bomb with (covering) iron. In other words the bomb wasn't closed 100%. A bomb that isn't closed(covered) very well with high temperatures can cause an explosion , but not fire. _____________________________________ Photo gallery : A military jet fighter managed to catch the planes last moments. Final BNSB report on the 3rd of May crashOn the 3rd of May, a Strato HMX 920 crashed into a mountain during a normal flight piloted by Captain Robert Gandy. The BNSB sent a team investigators who said they will make their final report next month and they did. Here is the report of one of the worst fatal accidents to happen in the Nevada Interstate from long time : Aircraft Accident Report : Controlled flight into terrain Private plane Strato HMX 920 Race Nevada Interstate 3rd of May / 2017 Abstract : This report talks about a crash that killed a pilot near the Nevada Interstate on the third of May / 2017, the pilot sustained fatal injures, and the racing plane was destroyed. The Team of investigators found that the banner connected to the plane was over the legal weight, that caused the plane's tail section to be damaged. The plane started to lose speed while flying near a mountainy area. The pilot tried to get more speed by making a sharp decent and so, but he was so unlucky on the 4th decent as the plane became face to face with a mountain. The pilot tried to pull as much as possible , but it was 2 late for him and the plane. The plane struck the mountain at a speed of 50 MPH which caused the plane to chatter into pieces. We also discovered that the pilot was in call with a military base ATC. Here are the last words made by the pilot and the ATC : Capt : Mayday Mayday ! BM 19 I'm losing control ATC : BM 19, roger can you tell me what would you like to do ? Capt : I want to crash land into any near highway ATC : BM 19 Roger, turn right heading 103 Capt : Are you sure ? Because there is a mountain on the way ATC : You said you wanted to crash into a highway, that's the only highway I can get on my radar Capt : Are you crazy ? I want to crash land in any highway and by highway I mean a road ATC : OK then, you are cleared to crash land into any road Capt : OH Sh*t, Its too late ! ___________________________*Sound of impact with the mountain* The investigators found that the ATC was confused between a road and a highway, the only highway name he had on his radar was a highway to the right where the plane was going. Furthermore, there was a mountain that the ATC couldn't see on his radar. The captain was the 2nd factor of the crash, he wasn't paying attention while turning right when he could basically turn left and crash land into a highway he was flying near by. His distraction caused him to see the mountain in the last few seconds when he couldn't save himself and the plane from crashing into it. __________________________________________________________________________________________________________________ (The plane involved in the accident, here it can be seen flying above the Nevada interstate in 1998) Date : 3rd of May / 2017 Site : Nevada Interstate Summary : Over-weighted banner combined with Spatial Disorientation and ATC confusion Aircraft type : Strato HMX 920 Race type Crew : 1 Fatalities : 1 Survivors : 0 Registration : BM-NGD 19 Name : Flying Wife
1953 Garfield Heights Disaster Date: July 9, 1953 Summary: Propeller separation leading to loss of control Site: Garfield Heights, Illinois, USA Total Fatalities: 3 Total Injuries: 0 Total Survivors: 0 (1 initially) ------------------------------------------------------------------------------- Aircraft: 86-9273 as she appeared 4 months prior to the accident, in Grooved Range. Type: North American B-25 Mitchell Registration: 86-9273 Passengers: 0 Crew: 3 Fatalities: 3 Survivors: 0 ------------------------------------------------------------------------------- Accident Summary: On a sunny, calm day in Garfield Heights, Illinois, the chartered aircraft took off from Runway 32L at James A. Garfield Airport at 9:15 AM. The flight was to continue to Grooved Range, Nevada after refuelling in Kansas. The pilots were cleared to climb to 10,500 feet (3,200 m), on a course set to North Garfield VHF omnidirectional range (VOR), which was reached at 4,650 feet (1,417 m). At that moment, the crew applied maximum thrust on the engines to climb to 10,500 feet (3,200 m). Supposedly, had they not applied thrust, the propeller shaft in the right (No. 2) engine would've survived the entire flight. However, 4 minutes after the thrust was applied, the faulty bearings overheated enough (to about 1,000 °C or 1,800 °F) to cause an explosion. Engine #2 exploded and started burning at 9:21:18 AM, as the aircraft has passed over James A. Garfield High School at 6,290 feet (1,917 m) at a speed of 370 km/h (229 mph). The overheated bearings exploded, destroying the shaft, the nacelle and propeller on the burning engine separated from the destroyed shaft; the freed shaft spun to an enormous speed, and within seconds, explosively disintegrated, destroying it. Debris from the explosion violently spread around (with an estimated speed of 160 metres per second (520 ft/s)), puncturing the fuselage, severing the flight controls cables and causing severe damage to the right wing, forcing the plane to the right. The pilots attempted to regain control over the stricken Mitchell via use of the left engine's throttle when the remaining control-ability was lost, causing the plane to enter a unrecoverable spiral descent. The crew radioed to JAG ATC that "we're going down hard". The plane impacted the ground inverted a full block from the Queen Street plaza, killing all 3 crew members as the aircraft broke up upon impact. ------------------------------------------------------------------------------- Cause: After the crash of the B-25C operated by the GHAF (Garfield Heights Air Force) due to a similar incident years earlier, the air force had started replacing the B-25s with a more efficent jet aircraft, and in the process of doing so, they have neglected to repair/service the B-25s that were still in service. According to the NTSB, the cause of the crash was the disintegration of an engine shaft due to faulty bearings inside the number 2 engine which seized, causing extensive heat. This in turn caused the consequent damage to engine number 2, loss of aileron control, and progressive hydraulic loss. The bearings concerned were roller bearings; each was designed to have 26 rollers inside, but because the supply of the rollers to the factory was delayed — while the bearings had to be finished on time due to expiring contracts — each bearing had only 13 rollers. ------------------------------------------------------------------------------- Aftermath: The GHFD was aware of the emergency; when the aircraft crashed, they immediately drove towards the crash site, and managed to extinguish all fires within 40 minutes following the crash. The day was warm and sunny, so many people rested in the vicinity of the impact; after the crash many of them headed for the site. According to official reports, the site was quickly closed to the public, and no plundering of remains was officially registered. However, some unofficial reports about stealing money and valuable items surfaced later; Katrina Garfield's personal belongings were stolen from her house, which was impacted by the severed empennage of the B-25. All victims' bodies were dismembered in the crash; all 3 victims were never identified. Due to the inverted impact, the bodies were destroyed almost instantaneously, combined with the force of the fuselage striking the cockpit, effectively sandwiching it, making it impossible to extract the remains. All crew members posthumously received high military and civil decorations. The state funeral of the crew was conducted on July 23, 1951 at the Hope Valley Cemetery in Garfield Heights. Two days of national mourning were announced after the crash. Many officials from the entire world, including Pope John Paul II, expressed their condolences to the families of the victims. ------------------------------------------------------------------------------- The crash site, less than 48 hours following the accident. An amateur photographer snapped this photo of 86-9273 shortly before impact. A special set of trained eyes may notice that the entirety of the right propeller, plus the nacelle is missing.
1994 Strato HMX 920 CrashOn the 2nd of June / 1994, a private Strato HMX 920 crashed while flying above Crimson Valley. The accident killed the pilot and left his plane destroyed. --------------------------------------------------------- Info : (a photo of the plane involved in the accident 1 month before it crashed) Site : Mountains / Crimson Valley Date : 2nd of June / 1994 Summary : Explosive Decompression caused by electric explosion due to faulty design Aircraft Type : Strato HMX 920 Crew : 1 Total fatalities : 1 Operator : Private Registration : 592 _________________________ Flight explosion : "Beautiful sky" pilot Eric said while he was flying his brand new Starto above Crimson Valley. "Everything seemed fine and calm" one police officer said. Suddenly, a loud noise was heard by everyone, at first, everyone thought it was a car accident or something similar , but it was the surprisingly a "plane that lost one of its parts" as reported by some hikers. It kept flying before it hit a mountain. The rescue team were informed, they managed to find the plane wreckage 15m after they started the search party. They found the pilot Eric alive , but he died on his way to the hospital. The Crimson Valley police asked the NTSB help to find what went wrong on that plane, the team arrived after 2 days. A huge search party was lunched to find the lost part from the plane. The search team managed to find the find the part miles away from the crash site. The lost part is surrounded with black. The investigators now know what caused the pilot to lose control. The team wanted to know what caused the plane to lose one of its most important parts. The investigators looked at the design of this Strato, they found that the the electric cables connecting the plane's engine to its part were way 2 close to the fuel tank. The fuel tank would explode if the engine starts to produce a lot of power. The fuel tank on that specific plane was near the seat glass cover. The investigators think that when the explosion didn't kill the pilot , but the high decompression levels and 2nd impact killed him. The NTSB said all Stratos must be checked and fix this problem as fast as possible before more accidents happen. _____________________________________________________- Photo gallery : Wreckage
